Go to the level called "Stopping The Fog" and go to the part where the Symbiote Machine is and go to the top. Find the hole and grab the Gold Spider-Icon.
Play as Carnage: At the Cheat menu enter code CARNAGE.
Play as Monster-Ock: At the Cheat menu enter code MONSTERO.
